The following is a list of question types currently supported by the Informis online survey
builder.

Text Area

- A large text-entry area, capable of accepting several rows of text.

Single Line Text
[SAMPLE]
- A single-line text-entry field.

Yes/No
[SAMPLE]
- Radio button selection, for asking dichotomous yes/no-type questions.

Yes/No/Unsure
[SAMPLE]
- Same as Yes/No above, but allows for respondents to answer "Unsure".

Likert Scale [SAMPLE]
- The classic Likert 7-point scale.

Five-Point Scale [SAMPLE]
- A 5-point rating scale.

Six-Point Scale [SAMPLE]
- A 6-point rating scale.

Seven-Point Scale [SAMPLE]
- A 7-point rating scale.

Select Box

- A drop-down select box, populated with client-specified options.

Grouped Single Line Text

- Single-Line text boxes grouped together.

Grouped Five-Point Scale [SAMPLE]
- For evaluating several concepts under a shared 5-point scale.

- As above, plus option to enter 'N/A' for null response.

- Allows the respondent to choose multiple answers from a list of options.

Constant Sum [SAMPLE]
- Continually calculates the sum of the respondent's numerical inputs. Serves as a guide for "Please divide 100 points between..." -type questions.

Zero-to-100-percent scale [SAMPLE]
- An incremental scale from 0 to 100% in ten-percent increments.
